Pulling Up Common Reports

Synergy EMR provides various reporting tools critical for operations and compliance. Below are the steps to access frequently used reports like Census Report, Medicare Cost Report, and ALIRTS/OSHPD (SIERA).


Census Report

Path:  Reports > Open Reports > Patient Reports > Census

Steps:

  • Select the Census report.
  • Adjust the Date Filter based on what you need:
    • Currently Active – includes patients with status: Under Evaluation, Admitted, or On Hold.
    • Active for a Specified Period – allows custom date range for when the patient was active.
  • Click Print to generate and preview the report.


Medicare Cost Report

Path:  Reports > Open Reports > Financial Reports > Medicare Cost Report

Steps:

  • Select the Medicare Cost Report.
  • Most users do not need to adjust filters.
  • Optionally, enter a custom date range if required.
  • Click Print to generate the report.


ALIRTS / OSHPD Report (aka SIERA Report for CA Agencies)

Path:  Reports > Open Reports > Statistical Reports > CA OSHPD Report

For Home Health Agencies:

Includes 6 sections:

  • Patients and Visits by Age
  • Discharges by Reason
  • Visits by Primary Source of Payment
  • Visits by Type of Staff
  • Admissions by Source of Referral
  • Patients and Visits by Principal Diagnosis

For Hospice Agencies:

Includes 14 sections:

  • Bereavement Services
  • Volunteer Services
  • Visits by Type of Staff
  • Unduplicated Hospice Patients by Gender and Age
  • By Gender and Race
  • By Gender and Ethnicity
  • Discharges by Reason
  • Discharges by Length of Stay
  • Admissions/Discharges by County and Disposition
  • Admissions by Diagnosis
  • Discharges, Visits, Patient Days by Diagnosis
  • Level of Care and Source of Payment
  • Location of Care Provided
  • Levels of Care 
  • Hospice Sites Provided

Important:  When generating the ALIRTS/OSHPD report:

  • All tables will open in separate tabs.
  • If only one table opens, your browser may be blocking pop-ups.
  • To fix: Go to your browser settings and allow pop-ups for your Synergy EMR site
